Malaise, weight loss and bone pain- what do you suspect?
Myeloma
Metastatic bone disease
Sclerotic bone lesions x ray
What conditions can be associated with bone sclerosis?
Metastatic prostate cancer
Metastatic breast cancer
Sickle cell disease - due to marrow hyperplasia and bone infarction.
Breast carcinoma can cause sclerotic, lytic or mixed bone lesions.
Prostate cancer usually causes sclerotic lesions and very rarely causes lytic lesions.
Why might smears be inadequate?
- failure to sample a full 360degrees of cervical circumference
- Presence of blood on smear
- Cervical inflammation
- Age related atrophic changes
Can leukaemia be associated with pv bleeding?
yes
Differentials for pregnant women coming it with intense pruritus of hands and feet - affecting sleep.
What investugations would you do if you suspected OC?
What are the risk factors for OC?
- Previous OC
- Asian origin
- Pruritis on COCP
- Genetic traits
Management and complications of OC
When are prophylactic antibiotics given in paediatric UTI?
With atypical UTI following an acute episode - until investigations are done to exclude VUR
PO prophylactic antibiotics following 7-10 days Iv abs
What features may be displayed in DDH?
What imaging techniques are used to test for DDH?
<5-6 months - USS of hips
>6mths- xray as bones have ossified
Which infants should receive screening at 6 weeks for DDH? - USS
- those with family history of first degree relative with hip problems early in life
- Breech presentation at 36 weeks - regardless of what presentation they were at delivery even if they had ECV
- Breech presentation at delivery (if before 36 weeks)
