A
- Immediate treatment with classic chemo/radiation
- Etoposide with Cis/carboplatin
- Amfostine and saline diuresis to limit platinum tox.

A
- Immediate treatment with surgical resection
- Chemotherapeutic treatment if resection is not fully possible or metastases
- Cisplatin with Paclitaxel/Docetaxel or Irnotecany/topotecan then premetrexed maintenance with Leucorvorin to minimize neurologic symptoms (hand/stocking)
