Q

Parkland Formula

A

4mL x %BSA x weight in kg = Total fluid in first 24hr

Give 1/2 over FIRST 8 hr

Remaining over next 16hr

Ventilator IBW Calculation

(For Male Pt)

A

Male = 50 + 2.3kg per inch > 60 = IBW

IBW x 6mL/kg = Vt

Q

Pediatric Resescutation 4/2/1 Rule

A

Use the Parkland Formula plus MAINTENANCE fluid

In 1 hour pt needs to get

4mL/kg for the first 10kg of body weight (0-10)

2mL/kg for the 2nd 10kg of body weight (10-20)

1mL/kg for remaining weight (> 20kg)

= mL/hr additional fluid

Q

100-50-20 Daily Fluid Maintenance

A

In one day a person needs:

100 mL/kg for first 10kg

50 mL/kg for 2nd 10kg

20 mL/kg for remaining weight > 20kg

Remember to divide by 24 to get hourly rate

Q

ET Tube Size Formula

A

1 y/o and older = 16 + age in years divided by 4 = ET size

or

Age in years divided by 4 + 4 = ET size

Preterm/fullterm = postconceptual age in weeks divided by 10

Q

ET Tube Depth

A

Suggested “lip to tip” 3 x tube size (oral)

4 x tube size (nasal)

or

Age in years divided by 2 + 12 = depth

Q

Cerebral Perfusion Pressure (CPP)

Calculation

A

MAP - ICP = CCP

CCP < 70 is associated with poor outcome brain injury

Systemic MAP is 60 - 180 mmHg

Q

Golden Rule for pH Changes

A

For every change in PaCO2 of 10 the pH moves 0.08 units in the Opposite direction.

For every change in HCO3 of 10, the pH moves .15 units in the Same direction.

For every decrease in pH of 0.1, the K+ changes 0.6 in the Opposite direction

Q

Medication Infusion Formula

mL/hr

A

Dose x weight x 60 / concentration

Ex Dopamine - 10mcg/kg/hr

Pt weighs 80kg

10x80x60 = 48,000 / 1600 (concentration)

= 30mL/hr

Q

Adjusted FiO2 Formula

A

Initial FiO2 x Initial barometric Pressure

divided by

Barometric Pressure at Cruising Altitude

Ex initial FiO2 = .70 at sea level (760mmHg) and cruising altitude will be 6,000ft (600mmHg) =

.70x760 / 600 = .88 % (adjusted FiO2)
