Calculation / Formulas Flashcards
Parkland Formula
4mL x %BSA x weight in kg = Total fluid in first 24hr
Give 1/2 over FIRST 8 hr
Remaining over next 16hr
Urine output .5mL/kg/hr
Consensus Burn Formula
2 - 4 mL x %BSA x weight in kg = fluid given in first 24hrs
1/2 given over the first 8hr
Remaining given over next 16hrs
2mL/kg Adults
3mL/kg Peds
4mL/kg electrical burns
Ventilator IBW Calculation
(For Male Pt)
Male = 50 + 2.3kg per inch > 60 = IBW
IBW x 6mL/kg = Vt
Pediatric Resescutation 4/2/1 Rule
Use the Parkland Formula plus MAINTENANCE fluid
In 1 hour pt needs to get
4mL/kg for the first 10kg of body weight (0-10)
2mL/kg for the 2nd 10kg of body weight (10-20)
1mL/kg for remaining weight (> 20kg)
= mL/hr additional fluid
100-50-20 Daily Fluid Maintenance
In one day a person needs:
100 mL/kg for first 10kg
50 mL/kg for 2nd 10kg
20 mL/kg for remaining weight > 20kg
Remember to divide by 24 to get hourly rate
ET Tube Size Formula
1 y/o and older = 16 + age in years divided by 4 = ET size
or
Age in years divided by 4 + 4 = ET size
Preterm/fullterm = postconceptual age in weeks divided by 10
ET Tube Depth
Suggested “lip to tip” 3 x tube size (oral)
4 x tube size (nasal)
or
Age in years divided by 2 + 12 = depth
Cerebral Perfusion Pressure (CPP)
Calculation
MAP - ICP = CCP
CCP < 70 is associated with poor outcome brain injury
Systemic MAP is 60 - 180 mmHg
Mean Arterial Pressure (MAP)
Calculation
Diastolic B/P x 2 + Systolic B/P / 3
Medication Injection Calculation
(mL to give)
Give divided by Have x mL in vial = mL you will give
Vital Capicity =
IRV + ERV + Vt = vital capicity
Minute Ventilation (Ve) =
Ve = Vt x RR = mL/min
Anion Gap Calculation
(Na+ + K+) - (Cl + HCO3) = Anion Gap
Normal 12 (+/-4)
Golden Rule for pH Changes
For every change in PaCO2 of 10 the pH moves 0.08 units in the Opposite direction.
For every change in HCO3 of 10, the pH moves .15 units in the Same direction.
For every decrease in pH of 0.1, the K+ changes 0.6 in the Opposite direction
Ventilator IBW Calculation
(For Female Pt)
Female = 45.5 + 2.3kg per inch > 60 = IBW
IBW x 6mL/kg = Vt
