Q
What are liver function tests, or LFTs?
A
- set of tests that check the health of the liver by measuring the enzymes and other substances made by the liver
2
Q
When are liver function tests used?
A
- when there are signs or symptoms of altered liver function:
- fatigue, nausea, vomiting, right upper quadrant pain, or jaundice - when monitoring clients with acute or chronic liver disease
- when checking liver function tests in clients taking hepatotoxic medications.
3
Q
What are the nursing managements for patients with altered LFTs?
A
Goals of nursing care: assist in identifying & treating any underlying cause.
- Collect a sample by venipuncture
- before collecting -> cnfm if they should be fasting before the test
- use a tube with a red top
- After obtaining sample, gently invert several times but dont shake the tube as this will cause hemolysis or breaking apart of the red blood cells -> alter results - If your client has elevated LFT:
- assess for signs of liver injury: RUQ abdominal pain, fatigue, nausea, or jaundice. - Check for other symptoms that indicate impaired liver function:
- easy bleeding or bruising could be related to decreased clotting factors
- If present: check prothrombin time, or PT, & international normalized ratio, or INR. - If edema is present
- check albumin level
- withhold any potentially hepatotoxic medications
- administer IV fluids and medications to treat the underlying cause, as prescribed. - Review their medication record to identify medications that can interfere with their test results
- acetaminophen, allopurinol, phenobarbital, phenytoin, oral contraceptives, salicylates, and statins.
