what is olecranon bursitis
inflammation of olecranon bursa (uid-filled sac overlying the olecranon process at the proximal end of the ulna)
cause of bursitis
trauma
* repetitive trauma
* direct trama
infection - staph aureus
RA / gout
idiopathic
features of bursitis
swelling over olecranon process - fluctuent and well circumscribed
tenderness
erythema
septic - pain and fever
skin abrasion
movement of elbow painless until swollen bursa is compressed in full flexion
ix for bursitis
clinical
aspiration for MCS if concern about septic
* purulent -> infection
* straw coloured -> non-infectious
mx of bursitis
avoid trigger
ice
compression to support area
elevate above heart -> reduce swelling
medical
* NSAIDS
* aspiration
* steroid inhections - persistent and non-septic cases - complication - infection and tendon rupture
surgery if -
* septic and not responding to rx
* recurrent -> functional impairment
