Factors that suggest inhalation injury
Incident-related
- Burn in a confined space
- Flash burns
Clinical examination
- Singed hair on the face
- Soot at the nostrils
Estimating burn area
Lund and Browder = most accurate
Wallace rule of 9s
Does not include areas of superficial burn
Referral to a burn centre
> 10% total burn surface area in adult
> 5% total burn surface area in child
Face, hands, feet genitalia
Burn involving airway inhalation
Circumferential
Very old frail patients
Pregnant patients
Chemical burns
Urine output for electrical burns
Aim 75mls / hour due to myoglobinuria
Parkland formula
= volume required in first 24 hours, split between first 8 hours and subsequent 16 hours
4 x TBSA in % x weight in kg = vol in mls
Recovery of burn without surgical intervention
At level of partial thickness burn
Dermal appendages intact
