Rule of Nines
Commonly used technique to determine burn size in adults
- Burn size is determined based on percentage of total body surface area (TBSA).
-Anterior & Posterior Arm: 9%
-Anterior Head & Neck: 4.5% (&post: 9%)
-Anterior Chest: 18% (&post: 36%)
-Legs: 18%
-Genitals: 1%
-Total Body Surface Area (TBSA): 31.5%
(Palmar Surface of hand: 1%)
The burn percentage TBSA is used for the following:
-Calculating nutritional and fluid requirements
-Determining level of acuity to establish the level of medical treatment needed (ex. admission to an intensive care unit)
-Classifying patients for use of standardized protocols
Phases of Burn Management and Rehab
Emergent Phase
From initial injury to approx. 72 hours after the burn
Medical Management: Emergent phase
-Attempt to stabilize
-May include: Dluid resuscitation, establishment of adequate tissue perfusion by mechanical ventilation, achieve cardiopulmonary stability
-Associated injuries also evaluated and treated here
Inhalation Injury (Emergent Phase)
Important consideration during this phase
-Damage too upper airway from inhaling smoke or noxious gases results in inhalation injury
-Damages the respiration
