Psuedomonas aeruginosa = 16%
Other chronic infection = 24%
Daily Sputum = 33%
Dry Bronchiectasis = 27%
Investigations for bronchiectasis
FBC, Ig levels Sweat test Sputum culture, including mycobacteria Serology for aspergillus species Spirometry
Treatable causes
CVID ABPA MAC infections TB Airway obstruction IBD RA Aspiration COPD
Treatment of Bronchectasis
Pulmonary rehabilitation
Pseudomonas eradication
Antibiotic therapy - 10 days
Physiotherapy for airway clearance techniques
Role of macrolides?
Immunomodulatory and antibacterial effect
Useful in patients with recurrent infections - decreases exacerbation frequency but nil change in disease course
Must exclude MAC colonisation
Azithromycin 500mg TDS
S/E: GIT pain and diarrhoea and QT prolongation
Pseudomonas eradication protocol
Ciprofloxicin 750mg BD for 2 weeks
–> 80% eradication
Decreases exacerbation frequency
