Breathing And Chest Trauma Flashcards
1
Q
Tension pneuomothorax
What is it and how to recognise
A
Puncture of pleura can lead to air entry.
Collapses ipsilateral lung
Marked respiratory distress Distended neck veins Reduced air entry ipsi Hyper resonant percussion ipsi Reduced chest expansion Tachy Cyanosis late Deviated trachea late
2
Q
Management of tension pneumo
A
Apply high flow o2
Immediate decompression with needle thoracocentesis
2nd intercostal space middle clavicular line. Just above third rib
3
Q
Open pneumothorax
Management
A
Open sucking wound
Breaches chest wall
IF hole in chest is greater than two thirds of the diameter of trachea air is sucked into cavity
ABCDE
high flow
Cover
Valve allows air to escape from thoracic cavity but will stop moving in
4
Q
Massive haemothorax
Versus haemothorax
Bleed into thoracic cavity
Reduced air entry, dull percussion note
A
Defined as 1500 ml or 30% of circulating volume
Recognise Tachyponea Tachycardia Ipsilateral rescued air entry Ipsilateral stony dull percussion note Possibly hypotension
