3 red flag symptoms that may have normal imaging
- nipple inversion - if new, unilateral
- Paget’s disease of nipple
- inflammatory breast cancer - a bit like mastitis, red, inflamed
6 Breast symptoms
- breast lump/lumpiness
- nipple discharge - bloody or clear fluid
- nipple change
- lump in axilla (can also get in bone)
- skin change
- pain (not usually breast cancer)
All breast symptoms seen within 2wks
What is the best treatment for small breast cancer lumps
Survival rates are same for lumpectomy and radiotherapy vs mastectomy
3 main categories of beast cancer drug treatment
- endocrine
- if cancer is oestrogen receptor positive use Tamoxifen
- chemotherapy
- use if oestrogen receptor negative of high risk oestrogen receptor negative
- designer drugs
- Herceptin for HER2 protein over expression
If oestrogen receptor negative and HER2 negative, considered triple negative breast cancer - these have worst prognosis
4 indications for mastectomy
- multifocal breast cancer
- local recurrence
- DCIS or local invasion >4cm
- BRCA1 or BRCA2 gene carrier - may request
1/3 have mastectomy
What age is breast cancer screening
Age 47-53, 3 yearly, organised by GP
1/2 present with symptoms, 1/2 through screening
What do you give if mother has breast infection while breast feeding
Encourage to continue breast feeding/ producing breast milk - stasis is not good and milk is fine for baby.
Give flucoxacillin - as it is fine for baby
