- mild to severe symptoms
- urinary frequency, urgency, nocturia
- hesitancy starting urination, weak stream, dribbling
- sensation of incomplete bladder emptying: > 100ml residual
- recurrent UTIs, increased BUN (chronic urinary retention)

- voiding diary, urinary flow rate, postvoidal residual urine
- digital rectal exam (DRE): large, rubbery, contender prostate
- urinalysis: hematuria, UTI
- elevated PSA, US, cystoscopy
- IPSS

- if obstruction: immediate catheterization, may need urologist intervention
- cystostomy: bladder incision (uncommon)

- pharmacologic therapy
