BPH
A progressive condition characterized by enlargement of the prostate gland commonly seen in males greater than 50 years of age
BPH Incidence
Affects 50% of men by age 50
Affects > 80% of men older than 80 years of age
BPH causes
Unknown; theorized prostate response to androgen hormones over time
BPH Sign/Symptoms
Frequency Dysuria Urgency Nocturia Incontinence Hesitancy “starting and stopping “urinary flow Dribbling Retention
BPH Lab/Diagnostics
UA: to detect infection Prostate-specific antigen (PSA) >4 abnormal; age-specific ranges are based on having had a PSA <4 40-49 <2.5 50-59 < 3.5 60-69 <4.5 70-79 <6.5
**Approximately 40% of patients with prostate cancer present with normal prostate specific antigen values
Transrectal ultrasound if there is palpable nodule or elevated PSA
BPH Managment
Observed condition and consult/refer to your all just as needed
Alpha blockers: Terazosin, prazosin,tamsulosin to relax muscles of the bladder and prostate
Five – alpha – reductase inhibitors: finasteride and dutasteride to shrink large Crossgates
Surgery if significant urinary symptoms persist: transurethral resection of the prostate
Saw Palmetto: may approve symptoms and some men; no evidence that it decreases prostate cancer risk
Avoid medication’s that were sent signs/symptoms of benign prostate hyperplasia or (antihistamines, decongestants, SSRIs)
