what are the 5 types of mechanical obstruction?
adhesions colorectal cancer insususeption hernia volvus
what is intususeption?
one segment of intestine “telescopes” inside of another, causing an intestinal obstruction (blockage)
where does intussusception commonly occur?
the junction of the small and large intestines
what is a Volvulous?
A volvulus is when a loop of intestine twists around itself and the mesentery that supports it, resulting in a bowel obstruction
what is the triad of symptoms in gastric volvulous?
- failed NG tube
- vomiting
- pain
what are causes pf pseudoobstruction?
myopathy- no peristalsis and food gets stuck
hishsprung disease- congenital issue- nerves missing at the distal end of the colon-> no peristalsis leads to bowel obstruction
how can bowel obstruction lead to sepsis?
- bowel becomes distended because food inside is unable to go anywhere
- distention squeezes surrounding blood vessels and reduces blood supply to that area of the bowel
- lack of oxygen leads to necrosis, further loss of peristalsis and an increase in anaerobic bacteria
- anaerobic bacteria is able to enter the circulation
what is the treatment for enraptured sigmoid volvulous?
decompression via rigid sigmoidoscopy and flatus tube insertion
what procedure would be preformed for obstructing sigmoid cancer/ perforated sigmoid ?
hartmans procedure
