Hand Hygiene
- Lathers all surfaces of wrists, hands and fingers producing friction for 20 SECONDS keeping hands lower than elbows and finger tips down
- Rinse all surfaces of wrists, hands and fingers keeping hands lower than the elbows and the fingertips DOWN
Apply One Knee High Stocking
- Stocking has no twists or wrinkles. Heel of stocking is correctly positioned Toe opening is either over or under toe area. (If using a mannequin, state that the sock is wrinkle free)
Assists to ambulate using transfer belt
- before assisting to stand client is wearing non skid socks
- before assisting to stand client is assisted to sitting position with feet flat on floor
Assists with use of bedpan (has no bold steps)
- places bedpan correctly under client
- REMOVE GLOVES WASHES HANDS
- raise head of bed
- give TP, handwipes & signal device
- PUT ON CLEAN GLOVES
- LOWER HEAD OF BED before removing pan
- Keep clients body covered
- empty/ rinse bedpan
- put bed pan in dirty supplies
- Remove and trash the Gloves
- WASH HANDS AGAIN
- give signal device lower bed
Cleans Upper and Lower Denture (has no bold steps)
- gloves
- line bottom of sink
- scrub all surfaces of dentures with toothpaste
- rinses cup lid and dentures with running water
- place dentures in cup with water in it
- rinses toothbrush and basin
- clean up
Counts and Records Radial Pulse
- Counts for 1 FULL Minute
- With in 4 beats of evaluator’s
Counts and Records Respirations
- Counts for 1 FULL Minute
- with in 2 breaths of evaluator’s
Dresses Client with Affected (weak) Right Arm
- Always dress the Weak arm first when putting clothes ON
Measures and Records Urinary Output (has no bold steps)
- pour urine into measuring container (no spilling or splashing)
- rinse off bedpan
- measure the amount of urine AT EYE LEVEL with container ON FLAT SURFACE
- Clean up
- remove gloves and wash hands BEFORE recording output
- Records with in 25ml of evaluator’s
Measures and Records weight of ambulatory client
- gait belt and skid
- scale is zeroed before client steps on
- Client steps on center of scale
- Client steps off
- WASH HANDS
- Records weight plus or minus 2lbs
Extension vs flexion
Dorsiflexion vs plantar flexion
Shoulder flexion/ extension
PROM for one knee and ankle
- Support the knee and ankle
- bend knee return to normal postion (3 TIMES)
- ASK PT IF IN PAIN
- Hold foor and ankle make ankle flex with toes up then point toes down (3 TIMES)
- ASK PT IF IN PAIN
PROM for one shoulder
- Support arm at wrist and elbow
- extension arm (straight down)
- flexion arm (Straight up near head)
- ARE YOU IN PAIN?
(Repeat 3 times)
- Support arm at elbow and wrist
- adduction arm (closest to body think add)
- abduction arm (away afrom body)
- ARE YOU IN PAIN?
(Repeat three times)
Postions on Side (has no bold steps)
- lowers head of bed
- side rail is up
- slowly roll client onto side
- fix head pillow
- Ensure patient is not laying on arm
- provide patient with back and side supports
- give patient pillow to “hug”
- place pillow between knees and ankles
Provides Foot Care on One Foot (has no bold steps)
- place water basin on a towel in front of resident
- Put on clean gloves
- Check water temp before washing foot (confirm with patient)
- Place patients foot in the water basin
- soap up washcloth lift foot out of basin, scrub foot & inbetween toes
- rinse foot and inbetween toes
- dry foot and toes
- apply lotion (exlcude between toes)
- Clean up
Transfer from Bed to Wheelchair Using Transfer Belt
- Make sure wheels on chair are locked
- Before patient stands ensure they’re in sitting postion with feet flat on floor
- (Not bold: make sure we use a signal on count of three etc)
Measures and Records Manual Blood Pressure
- Clean stethoscope (ear piece and diaphragm) with rubbing alcohol
- Wash hands before recording
- RECORDS WITH IN 8mm of evaluator’s
