β-blockers, aldosterone antagonists, diuretics Flashcards
β-blockers that are most useful in heart failure
Carvedilol.
Metoprolol
Bisprolol
β-blockers - when are they indicated for heart failure?
Heart failure caused by left ventricular systolic dysfunction.
Carvedilol - classification
Third-generation β1- and β2-blocker with α 1-blockade as well. Antioxidant, antiinflammatory, antiapoptotic.
Multiple-action neuroendocrine antagonist.
Carvedilol - effects
Increased left ventricular ejection fraction, slowed disease progression.
Does carvedilol improve symptoms of heart failure?
Yes
Does carvedilol improve survival in heart failure?
Yes
Carvedilol - indications in heart failure
Symptomatic heart failure in pts without significant hypotension, pulmonary congestion, AV block.
Carvedilol - adverse effects
Bradycardia, worsening heart failure, dizziness.
Aldosterone antagonists used for heart failure
Spironolactone and eplerenone
Aldosterone antagonists - effects in heart failure
Decrease plasma volume by increasing Na excretion, decrease K excretion.
Aldosterone antagonists - indications for heart failure
Severe heart failure
Aldosterone antagonists - contraindications
Renal insufficiency (hyperkalemia)
Aldosterone antagonists - adverse effects
Hyperkalemia, gynecomastia.
Diuretics used for heart failure
Loop diuretics (more preferred for heart failure) Thiazide diuretics
