Bleeding dz Flashcards
1
Q
Hemophilia
A
A: deficiency of F8, X-link
B: deficiency of F9, X-link
C: deficiency of F11, autosomal recessive
2
Q
Clinical symptom
A
Joint/Muscle bleeding: hard of walking (hard to notice in baby) CNS GI tract Urogenital tract Respiratory/Buccal Cavity
3
Q
Lab results
A
Prolonged PT, aPTT
Mixing test: should show correction for factor deficiency
4
Q
Treatment for hemophilia
A
- Factor replacement: prophylactic therapy, on-demand therapy
- Supportive therapy: ice, , rest, compression
- Emergency bleeding: F2 high concentrate, F7a, Anti-fibrinolysis
5
Q
Liver disease vs DIC (Disseminated Intravascular Coagulation)
A
Measure F5 (made by liver) and F8 (made by endothelial cell)
Only F5 decreased: Liver dz
Both F5, F8 decreased: DIC
6
Q
Approach patient
A
- History: bleeding, bruise, heavy mense, transfusion, famiy history, medication
- Lab: CBC, PT, PTT, FI, TT
7
Q
Vitamin K deficiency
A
- Requires for Carboxylation of F2, 7, 9, 10
- Dx by prolonged F7, correction with vitK
- Liver disease can affect because vitK react with factors in liver
- VitK made by gut flora –> newborn have no vitK
8
Q
Acquired bleeding dz
A
Liver dz, vitK deficiency, Anticoagulant therapy, DIC, amyloidosis, specific factor inhibitors
