* subjective report of depression—most of the day, nearly every day
* anhedonia
* weight loss
* insomnia or hypersomnia nearly every day
* psychomotor agitation or retardation
* fatigue
* feelings of worthlessness, excessive guilt
* indecisiveness
* recurrent thoughts of death
* suicidal ideation – with or without a specific plan

CB: significant distress or impairment
CC: not due to physiological effects of a substance, medical condition
