Benign Conditions of the Large Bowel *** Flashcards
1
Q
Describe the clinical features of diverticular disease.
A
- Generally asymptomatic
- Episodic colicky abdominal pain - sometimes constipation/diarrhoea
- Can progress to lower GI haemorrhage - in sudden, painless and severe bleeding from colonic diverticula
- Chronic blood loss leads to anaemia
2
Q
Describe aetiology of diverticular disease.
A
- High intraluminal pressure within the bowel
- Thinning of the bowel wall.
- Pouches of mucosa push between muscle fibres and develop into an outpouch
3
Q
Describe the morphological appearance within diverticular disease.
A
4
Q
Describe the gross appearance within diverticular disease.
A
5
Q
Describe the histological appearance within diverticular disease.
A
- Absence of muscle layer around herniated diverticula
- Thickened muscle layer of sigmoid colon
6
Q
Describe the complications of diverticular disease.
A
- Abscess formation
- Perforation leading to peritonitis
- Sepsis
- Fistulation
- Bowel obstruction
- Bleeding
7
Q
Define fistula.
A
- Tube with opening at both ends
- Abnormal communication between two surfaces is established.
8
Q
What are the 2 main types of fistula?
A
- CONGENITAL - due to congenital abnormality
- ACQUIRED - due to trauma, inflammation and necrosis
9
Q
Give an example of a disease where fistulas form.
A
Crohn’s
- Gut inflammation can spread through all the layers in the bowel wall.
- Cause small leaks and abscesses to form
- Increased risk of UTIs
