- Young women
- Underlying problem is periductal mastitis
- Presents as inflammation +/- mass, abscess or mammary duct fistula
- Aerobic and anaerobic organisms
- Nearly always associated with cigarette smoking
- Hx of recurring problems and prior abx or incision and drainage; up to 50% recurrent

- Antibiotics (broad spectrum aerobic and anaerobic coverage)
- Aspiration if needed
- QUIT smoking

- surgical excision of subareolar ducts after acute infection is controlled
