Eng check control panel to identify space, notify cox and silence alarm

Follow up actions

Eng and crew investigate space if safe
Crew will prepare basic casualty procedures ( dc kit and p6 pump)
Crew check material condition of all spaces informing cox
Determine if safe to proceed with mission or RTB

Loss of control of Engine RPM

A

Initial

Bring throttles to clutch ahead
Notify crew

Follow up

Cox insures throttles station is active and sync is off
Crew shift to another station and attempt to gain throttle control
Use emergency backup panel
Use engine stop button to secure effected engine
Eng pulls fuel cutout for affected engine
Eng enters aux space and secures affected engines DDEC breaker on 24 power panel
Cox uses emergency air shut down

Loss of fuel oil pressure

A

Initial

  • Reduce to clutch ahead determine which engine is affected
  • Inform crew, verify position, evaluate situation notify station

Follow up

  • Crewman rigs anchor
  • engineer to engine room asses before entering
  • Eng enters with crewman as safety with fire extinguisher
  • engineer check bilge for fuel oil
  • Eng check emergency fuel cutout valves
  • Eng check primary fuel filters for sediment and or water in bowl replace if suspect re prime
  • Eng check entire fuel system for obvious leaks; check fuel tank level
  • Eng identifies problem or requests further assistance
  • coxswain RTB with one engine and if for 30 min or more closes the exhaust cross-over valve

Loss of lube oil pressure

A

Initial

  • cxc comes to clutch ahead determine affected engine
  • cxc immediately secures affected engine
  • notify crew, verify position, contact station

Follow up

-crewman rigs for anchoring
-Eng proceed to engine room asses before entering
-Eng enters engine room with crewman as safety
-crewman checks bilge
-Eng checks for quality/quantity
-Eng checks recovery tank for contamination
Note if not correctable do not restart engine; RTB on one engine

Main engine high water temp

Cxc reduces to clutch ahead/determine affected engine
Cxc notify crew verify position and contact station
Cxc secures engine of temp exceeds 212

Follow up

Eng head to engine room look thru port hole to assess situation
Eng enters engine room with crewman as safety
Eng check engine temp on mechanical gauge, check bilges and engine for obvious leaks
Eng feels R/W brass to see if cool to touch or hot to determine what system casualty is in
Eng does system intact check for Raw water system

Reduction gear failure

Initial

Cxc brings throttles to neutral notify crew
Verify current position, evaluate situation and notify station
Cxc checks EDM for R/G pressures disengaged 58-66 engaged 230-290

Follow up

Ensure active light is lit at control station
Cxc shifts station to try to regain control or engaging back up panel
Cxc secures affected engine
Crewman rigs anchor
Eng checks EGIM breakers on 24volt panel
Eng checks affected R/G lube oil level and bilge for oil
Eng checks DCV electrical connections
Eng checks dirty oil filter indicator and if pooped handle shall be shifted to opposite strainer
Restart engine if no leaks present and oil is full and check clutch applied pressure secure if out of parameters
If not mechanical check DDEC SYSTEM
Cxc and eng discuss using come home device

Unusual noise or vibration

A

Initial

Cxc comes to clutch then to neutral
Notify crew verify position notify station
Eng lays to engine room assess situation then enter with crewman as safety

Follow up

Cxc comes to clutch ahead while Eng listens/looks for vibrations or noise
Cxc increases rpm’s by 50 and if vibration is detected safe speed is 200 rpm’s below the rpm vibration or noise was detected
