IVP: 5-20mcg Q3-5 min Iv push after Sailine Bolus. Saline bolus size will depend on if it is cardiac and trauma related.
Infusion: 2-10mcg/min

To mix infusion
Mix 1ml of 1:1,000 with 100ML of NS (12-60 ml/hr)
Use 60 drop set

2mcg/min = 12ml/hr
3mcg/min = 18ml/hr
4mcg/min = 24ml/hr
5mcg/min = 30ml/hr
6mcg/min = 36ml/hr
7mcg/min = 42ml/hr
8mcg/min = 48ml/hr
9mcg/min = 54ml/hr
10mcg/min = 60ml/hr

To mix pressor bolus
Mix 1ml of Epi 1:1,000 with 100ml of NS.
20 mcg = 2ml

Allow lidocaine to swell in IO for 60 seconds
Flush w/ 5ml of NS
Repeat with half of initial dose given over 60 seconds
