What is a Bartholins cyst?
Cyst or abscess in a bartholin/s gland.
What is the aetiology and epidemiology of a Bartholins cyst?
Obstruction to duct leads to accumulation of fluid in gland and distension.
2% women, usually 20-30.
What is the history and exam of a Bartholins cyst?
Unilateral vulval swelling. Painles cyst. If abscess, can cause pain, difficulty walking and sitting, dyspareunia.
Unilateral swelling in Bartholins glands region. Abscess will be tender, fluctuant, warm, erythrematous and surrounding cellulitis may be present.
What is the pathology of a Bartholins cyst?
Superinfection predominantly with Strep, Staph and E coli.
What investigations do you do for a Bartholins cyst?
Microbiology (cyst/fluid cavity MCS)
What is the management of a Bartholins cyst?
Small painless cyst: conservative.
Small abscess: may respond to ABX
Large abscess: require marsupialisation (incision of gland and formaiton of tract from interior wall to exterior.
What are the complications/ prognosis of a Bartholins cyst?
Recurence rate 20%.
