Enterococcus faecalis?
known as Group D Strep, so gram + , catalase -ve (staph have cats, streps don’t!)
lives in GI naturally (thats why entero) since lives in GI, fermenter (all GI bacs are fermenters)
commonly cause of UTIs and nosocomial infections since Strep group, can also cause subacute bacterial endocarditis (usually those first colonized by GA Strep, to make it easy- opportunistic!), but Strep viridans is most likely suspect in Strep endocarditis
class notes: treat with ampicillin (IV - want stronger since opportunistic) penicillin class antibiotic - works against bacterial cell wall AND gentamycin (aminoglycoside for synergy)
cards: inhabits intestines, biliary tract, genitourinary tract -> overgrows locally when normal flora is suppressed by antibiotics, esp. cephalosporins -> dah, nosocomial bacterial infection likely! (UTI, biliary tract), also reason why want ampicillin (IV) not oral to make sure it goes away. can also treat with vanco.
