- How would you begin and conduct your visit? Max 8
- Introduce - name and role
- Tell what going to do today.
-daily examination of alfie
- consent-including undressing alfie
-head to toe examination - looking for deviations from normal
- weigh baby using local guidance
- support parents/ answer any questions - Look out for any deviations from normal - pass on to midwife
- Refer on if necessary
- Newborn wellbeing
- Support and advice to parents in new role
- What would be your initial observations be when entering the house? Max 10
- Is environmental safe?
- Temperature of room
- Smoky?
- Any pets running around
- Are parents interacting/holding with baby
- Do the parents look/appear to be well?
- Home clean/tidy - overwhelmed?
- Give advice for any concerns
- How would you prepare for examination of baby? Max 8
- Wash hands - WHO 5 moments of hand hygiene 7 steps of hand washing - use gel if visibly clean - prevent cross infection
- Apply PPE - gloves and apron
- Make sure room warm - if not reduce time of exposure
- Good lighting
- Warm hands
- What info would be helpful prior to examining baby? Max 21
- Baby’s behaviour
- Has baby been sleeping?
- How is baby feeding and how often? - both breasts and are they being emptied at feeds?
- Is feeding going well/volume of feeds?
- How many wet (5-6 - 24 hours)/dirty nappies(min 2 - 24hours)- size of 2pound coin?
- Change in colour/texture - starting to become lighter?
- Concerns?
- Does baby look well?
- How is it lying in cot/reducing SIDS - feet to foot-no bumpers or toys in cot
- Is it dressed appropriately/ no hat if they are indoors?
- Resps 30-40 rpm - 60 if crying
- Chest symmetric
- Nostrils flaring/chest drawing in-overworking
- Vomiting/positing
- Limb movement-all 4

Feel fontanelles and suture lines
Move head look for trauma
-calput or moulding
Forceps trauma
Cephal haemotoma
Nose
Nostrils patents

White part colour? White or yellow
Scalera clear-no bloodshot or trauma
Mouth?
Clear moist
No thrush or infection
Lip sucking blisters

Folds - no infection
No excoriation
Look out for rashes-give advice
Hygiene-positing-neck has been cleaned and dried thoroughly
Skin
Check skin- colour appropriate for ethnicity jaundice - test if needed
Rashes/infection/spots- different in different skin colours
Birth mark - noted already
Good peripheral perfusion-no circulation challenges
No cyanosis
Arms and legs
Folds
10 fingers and toes- no webbing
Nails- paramecia
Can all limbs move
Infection/poor hygiene
Umbilicus
Signs of separation/challenges with separation
Clean dry-parents advice
No signs of infection
Not covered by nappy
Groin and buttocks
Looks at skin for rash/trust/nappy rash
Change nappy if needed
Anus patent
Genitals as should be
Weight
5 to 10% loss of birth weight

Bladder (Uris acid)
Check nappy for urates - normal for 3 days after birth
Feeding gets rid of urates
