Biceps brachii
Long head: Supraglenoid tubercle and superior portion of glenoid labrum to radial tuberosity and bicipital aponeurosis
Short head: coracoid process to radial tuberosity and bicipital aponeurosis
Brachioradialis
Upper lateral supracondylar ridge of humerus to superior aspect of styloid process of radius
Supinator
Lateral epicondyle of humerus and radial collateral ligament to proximal portion of anterolateral surface of radius
Triceps
Long head: infraglenoid tubercle of scapula to posterior surface of olecranon process of ulna
Lateral head: superior half of posterior surface of humeral shaft to posterior surface of olecranon process of ulna
Medial head: posterior shaft of humerus, distal to the spiral groove, to posterior surface of olecranon process of ulna
Pronator teres
Humeral head: superior portion of medial epicondyle to lateral aspect of mid shaft of radius
Ulnar head: coranoid process of ulna to lateral aspect of mid shaft of radius
Brachialis
Lower half of anterior aspect of humerus to tuberosity and coronoid process of ulna
