Inhibits acetylcholine increasing heart rate
Indications
Bradycardia with hypotension Organophosphate poisoning Nerve agent exposure RSI in pediatrics Beta blocker overdose Calcium channel blocker overdose
Contraindications
Tachycardia
Unstable cardiovascular status in acute hemorrhage with myocardial ischemia
Narrow-angle glaucoma
Hypothermic bradycardia
Side effects
Tachycardia Headache/confusion Palpitations Dysrythmias Blurred vision Flushed/hot dry skin Decreased oral secretions
What happens if pushed too slowly or given at low doses?
Paradoxical bradycardia

1mg to 10mL
0.1 mg/mL
Prefilled syringes
Adult dose
Unstable bradycardia
0.5 mg/kg IV or IO every 3-5 minutes
Maximum: 0.04 mg/kg or 3mg total (6 doses)
Pediatric dose
Unstable bradycardia
0.02 mg/kg IV/IO
Minimum: 0.1mg
Maximum single: 0.5mg dose (can repeat once)
Maximum total: 1mg
Adult organophosphate poisoning
2-4mg
Pediatric RSI
0.01-0.02 mg/kg IV/IO
Minimum: 0.1mg
Maximum: 0.5mg
Peak effect
1-2 minutes
Duration
2-6 hours
Adolescent dose
Unstable bradycardia
0.5mg IV/IO every 3-5 minutes
Minimum dose: 0.1mg
Maximum single: 1mg
Maximum total: 3mg
