in moderate / severe asthma exacerbation, how often can albulterol be given?
q 1 hour or continuous SABA
how can pt objectively tell they need SABA at home?
PEF 50-79%
at home, pt determines need for SABA. How use properly?
up to 2 treatments 20 min apart of 2-6 puffs by MDI or neb. Then may continue SABA q 3-4 h x 48 hrs
pt has incomplete response to SABA: PEF 50-79%. Now what?
steroids
pt used SABA, but PEF
go to ER

what must be taken into consideration with PEF
Age, sex, height
define asthma exacerbation by PEF
Decrease from pt’s baseline by 20% or more
albuterol MDI (90 mcg/puff)
4-8 puff q 20 min up to 4 h, then q 1-4 h prn
levalbulterol dosing (45 mcg/puff)
same as albuterol MDI
