Internal transvaginal ultrasound
Invasive procedure in which a probe is inserted into vagina to allow for a more accurate evaluation. It does not require a full bladder. Useful for clients that are obese, and those in first trimester to detect ectopic pregnancy, identify abnormalities and gestational age.
Doppler ultrasound blood flow analysis
A non invasive external ultrasound method to study the maternal-fetal blood flow by measuring the velocity at which RBCs travel in the uterine and fetal vessels. It is extremely useful in fetal intrauterine growth restriction (IUGR) and poor placental perfusion. Also pregnancies at risk because of HTN, DM, multigravida, or preterm labor.
Nursing actions for ultrasound
Advise patient to drink 1 to 2 quarts of fluid prior for full bladder.
Biophysical profile BPP
Uses a real time ultrasound to visualize physical and physiological characteristics of the fetus and observe for fetal biophysical responses to stimuli.
Total score of 8-10 is normal
Nonstress test
Most widely used technique for fetal well being technique, evaluation performed during 3rd trimester.
Alpha-fetoprotein AFP
Can be measured from the amniotic fluid between 16-18 weeks of gestation.
Blood test that ascertains info about the likelihood of fetal birth defects.
Low levels of AFP can indicate a risk for Down syndrome
High levels of AFP can indicate a risk for neural tube defects
External abdominal ultrasound
Safe, noninvasive and painless procedure where it obtains an image of abdominal contents
Spontaneous abortion
When a pregnancy is terminated before 20 weeks of gestation or a fetal weight less than 500 g
Gestational trophoblastic disease- Hydatidiform Mole, choriocarcinoma, and molar pregnancy
Low carotene or animal fat intake
Age- early teens or over 40
Ovulation stimulation w clomid
S/s- excessive vomiting due to elevated hcG levels

Rx for client in preterm labor, ca channel blocker used to suppress uterine contractions
S/E dizziness/assoc w orthostatic hypotension, facial flushing, heat sensation, nervousness, jitteriness, sleep disturbances
Methergine
Ex for pp hemorrhage, contracts uterus
Group B streptococcus
Bacterial infection that can be passed to fetus during labor
Maternal age less than 20, African American or Hispanic descent
Prolonged rupture of membranes
