BIOPSYCHOSOCIAL-SPIRITUAL-CULTURAL HISTORY
Information on the current/presenting issue or issues; a past and present physical health, developmental milestones, emotional functioning, educational/vocational background, cultural issues, spiritual and religious beliefs, environmental issues, social functioning.
BIOLOGICAL
assesses a client’s medical history, developmental history, current medications, substance abuse hist., family history of medical illnesses.
PSYCHOLOGICAL
assess a client’s present psychiatric illness or symptoms, past or current psychosocial stressors, and mental status
SOCIAL
focuses on client systems and unique client context and may identify strengths, and/or resources available for treatment planning
PSYCHOSOCIAL STRESS
Results when there is a perceived threat (real or imagined)
Stress manifests via high blood pressure, sweating, rapid heart rate, dizziness, and/or feelings of irritability or sad
DIFFERENTIAL DIAGNOSIS
a systematic diagnostic method used to identify the presence of an entity where multiple alternatives are possible
COMORBID
existing with or at the same time; ex. Having two different illnesses at the same time
CONTRAINDICATED
not recommended or safe to use (a medication or treatment that is contraindicated would not be prescribed b/c it could have serious consequences)
DELUSION
false, fixed belief despite evidence to the contrary (believing something that is not true)
DISORIENTATION
confusion with regard to person, time, or place
DISSOCIATION
disturbance or change in the usually integrative functions of memory, identity, perception, or consciousness (often seen in clients with a history of trauma
ENDOGENOUS DEPRESSION
depression caused by a biochemical imbalance rather than a psychosocial stressor or external factors
EXOGENOUS DEPRESSION
depression caused by external events or psychosocial stressors

HYPOMANIC

subsequent to the onset of an illness
PREMORBID
prior to the onset of an illness
PSYCHOTIC
experiencing delusions or hallucinations

TARDIVE DYSKINESIA
ABNORMAL, INVOLUNTARY MOVEMENTS OF THE TONGUE, LIPS, JAW FACE AS WELL AS TWITCHING AND SNAKE LIKE MOVEMENTS OF THE EXTREMITIES. MAY RESULT FROM TAKING HIGH DOSES OF ANTIPSYCHOTIC MEDICATIONS OVER A LONG PERIOD OF TIME

antianxiety drugs
used for the treatment of anxiety disorders
avitran, buspar, klonopin, valium, xanax
Benzodiazepines
class of drugs primarily used for treating anxiety, also effective in treating other conditions.
*High abuse potential of these drugs, can be dangerous when mixed with alcohol or illicit substances. Look for signs of impaired motor functioning, other functioning
Stimulants
Used for the treatment of Attention-Deficit/Hyperactivity Disorder
Adderall, Concerta, Dexedrine, Metadate, Ritalin
Advair Diskus
prescription used to treat asthma and chronic obstructive pulmonary disease (COPD)
Crestor
ipid-lowering agent taken orally
Cymbalta
selective serotonin and norepinephrine reuptake inhibitor (SSNRI) for oral administration
diovan
used to treat heart disease of heart failure
Hydrocodone/Acetaminophen
most popular painkiller used to treat moderate to severe pain. Narcotic analgesic, relieves pain through the central nervous system, also used to stop or prevent coughing. Can become habit forming, when used over an extended period of time
Levothyroxine sodium
used to treat hypothyroidism, condition where the thyroid gland does not produce enough of the thyroid hormone. Also used to treat thyroid cancer and to help shrink an enlarged thyroid gland.
Lantus
sterile solution of insulin glargine for use as a subcutaneous injection for diabetes
Lisinopril
(used to be Zestril and Prinivil), high blood pressure medication. Main function is to block chemicals in the body that trigger the tightening of blood vessels. Lisinopril also is used to help treat heart failure.
Lyrica
used to control seizures, as well as treat nerve pain fibromyalgia.
Metoprolol
(generic form of Lopressor) used to treat high blood pressure and also helps reduce the risk of repeated heart attacks. Metoprolol also treats heart failure and heart pain or angina.
Nexium
used to treats symptoms of gastroesophageal reflux disease (GERD) and other conditions involving excessive stomach acid
Simvastatin
(generic form of Zocor) is prescribed to treat high cholesterol and is typically recommended in conjunction with diet changes. This drug is believed to have a variety of benefits including helping prevent heart attacks and strokes
