When using possessives for family, in the singular, you don’t need the article, with the plural, you do. With loro (they) you always use the article.

mio zio → il miei zii

tuo cugino → i tuoi cugini

sua sorella → le sue sorelle

nostra cugina → le nostre cugine

vostra madre → le vostre madri

il loro fratello → i loro fratelli

Also, if the noun referring to a family member is modified by an adjective or a suffix (like -lino meaning little), the article is retained.
