Who is exempt from licensure in AZ?
1) Student on clin ed operating under someone else’s license 2) PT or PTA who works in the armed services, public health service or veterans admin 3) PT licensed elsewhere who is performing PT in connection with teaching for not more than 60 days in 12 month period 4) PT licensed elsewhere who is working for athletic teams, organization or performing arts if not more than 60 days in calendar year 5) PT licensed elsewhere who is helping victims of a local, state or national disaster (not more than 60 days)
Requirements for licensure (educated in US)
1) complete application 2) graduate accredited PT ed program 3) passed NPTE 4) passed jurisprudence exam 5) valid fingerprint clearance card
Requirements for licensure (if trained outside US)
1) complete application 2) show evidence that education is equivalent to US 3) proof that able to practice in home country 4) provide proof that able to reside and work in US 5) English proficiency exam 6) participated in supervised practice 7) pass NPTE 8) pass jurisprudence exam 9) valid fingerprint clearance card
Reasons for denying an applicant for PT/PTA license
1) making a false statement in any application 2) committing fraud 3) committing a felony or misdemeanor involving moral turpitude (court conviction) 4) engaging in conduct that subverts or undermines the exam or exam process
What does a new grad need to apply for licensure?
1) on official letterhead that student is a candidate for PT or PTA degree with grad date 2) Can sit early, on date closest to graduation date but not more than 120 days prior 3) grad meets all of programs requirements
Can board exam be re-taken?
1 retake within 6 months of original date without reapplication
When is an interim permit given?
1) Foreign-educated applicant for participating in supervised clinical practice 2) Board determines applicant lacks competence to practice, can be given interim to participate in supervised clinical practice 3) for at least 90 days but not more than 6 months
When is a license suspended?
1) fails to renew the license before expiration date 2) applicant doesn’t submit a complete application or pay renewal fee
When is a license reinstated?
Fee paid, reinstatement fee paid and completion of the application (if under 3 years)
What must someone do if their license has been expired for more than 3 years?
1) practice under an interim permit 2) Complete remedial course 3) Completing continuing competence requirements for the period of lapsed license 4) Passing an exam
What are the license fees?
$300 for original license, certificate renewal, reinstatement of license; $50 for duplicate license
What does the a business entity need to do to apply?
1) describe entity’s services 2) manager’s name 3) names and addresses of the officers and directors 4) pay registration fee
How often does a business entity need to re-register with the state?
Every odd numbered year on August 31. Need to submit an application.
Under what conditions must the business entity notify the board in writing?
1) Change in business name, address or phone number 2) change in officers or directors 3) name of manager
