  1. High disease activity

2. Previous use of DMARDs

17
Q

What conditions are contra indications for anti TNF medication?

A

Heart failure

Pulmonary fibrosis

18
Q

How is infliximab given?

A

IV

19
Q

What biologic can be used in connective tissue disease?

A

Rituximab

20
Q

Ustekinumub

21
Q

22
Q

How would you treat an acute attack of gout?

A

Colchicine
NSAIDS
steroids

23
Q

What medicine could you start for a man who has had 4 gout flare ups in the last year?

A

Allopurinol. Co prescribe this with an NSAID for the first 4 weeks.
If this doesn’t work, try febuxostat.

24
Q

How does allopurinol work?

A

It is an xanthine oxidase inhibitor and so decreases the metabolism of xanthine into uric acid. This stops the levels getting too high and stops uric acid crystals from accumulating in the joint.

25
Q

How long should you wait after a gout attack to start allopurinol?

A

2 - 4 weeks

26
Q

Why should you not start allopurinol during an acute attack of gout?

A

Rapid reduction in uric acid levels may result in an exacerbation of the gout attack.

27
Q

What disease makes problems with allopurinol more likely?

A

Renal impairment

28
Q

What drug has a potenitially fatal result if taken with allopurinol

A

Aziothriorine. Never co prescribe these!!!!!!!

29
Q

Are corticosteroids lipophillic or lipophobic?

A

Lipophillic

30
Q

What HLA allele is strongly linked to ankyolising spondylitis?

A

HLA B 27

31
Q

32
Q

A

Swan neck deformity

34
Q

What deformity is this: Hperflexion at the PIP joints and hyperextension at the DIP joints?
