Five recommended initial regimens
- BIC/TAF/FTC (Biktarvy)
- DTG/ABC/3TC (Triumeq)
- DTG + (FTC or 3TC) + (TAF or TDF)
- -Could be TAF/FTC (Descovy) or TDF/FTC (Truvada) or DTG/3TC (Dovato) + (TAF or TDF) - DTG/3TC (Dovato) - EXCEPT HIV RNA > 500K, HBV co-infection, or starting before resistance genotype)
- RAL + (FTC or 3TC) + (TAF or TDF)
Recommended initial regimen for most people
INSTI + 2 NRTIs (or INSTI +1 NRTI)
Second line regimens
- Boosted PI + 2 NRTIs: (DRV/c or DRV/r > ATV/c or ATV/r) + (TAF or TDF) + (FTC or 3TC)
- -DRV/c/FTC/TAF = Symtuza - NNRTI + 2 NRTIs: (DOR or EFV or RPV) + (TDF or TAF) + (3TC/FTC)
- -DOR/TDF/3TC = Deltsrigo
Regimen when ABC, TAF, TDF cannot be used
DTG/3TC - if HIV VL <500K, resistance pattern available and no HBV co-infection
DRV/r + RAL BID - if HIV RNA <100K and CD4 >200
DRV/r + 3TC
Cannot use what two regimens if CD4 <200
RPV-based regimens
DRV/r + RAL
(Higher rate of virologic failure)
Cannot use what three regimens if HIV RNA > 100K
RPV-based regimens
ABC/3TC + (EFV or ATV/r)
DRV/r + RAL
(higher rates of failure)
Cannot use what 4 regimens if HIV RNA >500K
RPV-based
ABC/3TC + (EFV or ATV/r)
DRV/r + RAL
DTG/3TC (little data available)
Cannot use what drug with HLA-B*5701
ABC - hypersensitivity, potentially fatal
What 3 regimens to avoid if starting ARV before resistance testing
