Arthrocentesis and Synovial Fluid Analysis:
Gram Stain
Culture
Leukocyte Count with differential
Wet prep for crystals

CBC sensitivity 60% for non gonococcal septic arthritis
CRP, ESR for monitoring treatment
Lyme titer
Rheumatoid factor
Antinuclear antibodies
HLA-B27
Lupus anticoagulant

Approach to knee aspiration

A

Medial or Lateral approach
Prep with alcholol
Anesthetize with Xylocaine
Prep with chlorhexidine
Prep sterile field
Identify the midpoint of the patella
Insert 18 g needle 1 cm inferior to the patellar edge
Direct the needle posterior to the patella and horizontally towards the joint space

A

Overlying cellulitis
Coagulopathy
Consult ortho for aspirating prosthetic joints
