Characteristics of murmur
A
Ejection systolic, loudest in the aortic area, louder on expiration, radiates to the carotids

Clinical signs of severity
A
- Low volume, slow rising pulse with narrow pulse pressure
- Reversed splitting of the second heart sound
- Soft aortic component of the second heart sound
- Signs of pulmonary hypertension
- Signs of pulmonary congestion

ECHO findings in severe AS
A
- Valve area <1.0cm2
- Vmax (peak transvalvular velocity) >4.0m/s
- Mean pressure gradient >40mmHg

Indications for valve replacement
A
- Severe AS with symptoms
- Moderate/severe AS undergoing cardiac surgery
- Severe AS and LVSD/abnormal BP in response to exercise, VT, valve area <0.6cm2

TAVI or surgical valve replacement
A
TAVI in >75yo or unsuitable/high risk for surgical valve replacement
