Antiulcers?
Antacids H2 blockers PPIs Sucrafate Misoprostol
Antacids? What do they do? What are their side effects? What are the nsg?
Aluminum hydrox, Sodium bicarb, Ca carbonate (most effective), mag hydroxide
Neutralize hydrochloric acid. Dont coat the ulcer
SE: anorexia, constipation, weakness, flatulence
Nsg: Monitor for hypercalcemia & hypermagnesemia
Ideally taken 1-3 hrs after meals
H2 blockers? What do they do? What are the side effects? What are the nsg?
Cimetidine, Famotidine, Rantidine
Block h2 receptors which then reduces gastric secretion & concentration
SE: HA, dizziness, constipation, pruritus, rash, decreased libido, impotence
Nsg: Report pain, coughing/vomiting of blood
Monitor BUN, creatinine, & alk phos (increases them)
Use w/caution w/PO anticoags
Before meals or at bedtime
PPIs? What do they do? What are the side effects? What are the nsg?
Omeprazole, Esomeprazole, Pantoprazole, Lansoprazole
(proton pump inhibitors)
Suppress gastric acid secretion by inhibiting the hydrogen/K+ ATPase system. Blocks up to 90% more gastric secretion compared to H2’s
SE: HA, dizziness, diarrhea, abd pain, rash
Nsg: Use w/caution w/digoxin, oral anticoags, phenytoin, & some benzodiazepines
Take BEFORE meals
(used to treat zollinger ellison syndrome)
Sulcrafate? What does it do? What are the side effects? What are the nsg?
Sulcrafate (Pepsin inhibitor)
Covers & protects the ulcer; does NOT neutralize/decrease acid
SE: constipation, flatulence, dry mouth, pruritus, nausea, HA
Nsg: Give on empty stomach
Therapy typically 4-8 weeks to heal ulcer
Caution w/renal impairment, tetracyclines, phenytoin, cipro
Misoprostol? What does it do? What are the side effects? What are the nsg?
Misoprostol (Prostaglandin analogue)
Suppresses gastric acid secretion & increases protective mucous
SE: diarrhea, abd pain, chills, hyperthermia
Nsg: Pregnancy Cat X!!!
Usually given if pt is on long term NSAIDs
