Antipsychotics and Anticholinergics Flashcards
Haloperidol Classification
1st Gen Antipsychotics
Haloperidol Therapeutic Effect
dopamine antagonist - emotional quieting
tx only positive symptoms of Schizophrenia (hallucinations/delusions)
⬇️ Norepinephrine - mania, anxiety, aggression
Haloperidol Side Effects / Adverse Effects
EPSEs - Extrapyramidal SE
Amenorrhea, galactorrhea (women), gynecomastea (men) from blockade of dopamine with increased prolactin
Blurred vision, dry mouth, urinary hestitancy, constipation from blockade of acetylcholine
Orthostatic hypotension from blockade of muscarinic receptors
Weight gain and sedation from blockade of histamine
Neuroleptic malignant syndrome - decreased consciousness and responsiveness, increased muscle tone and autonomic dysfunction caused by excessive dopamine receptor blockade. Fatal in 10% of cases
Clozapine Classification
2nd Gen Antipsychotics (SGA)
Clozapine Therapeutic Effect
Emotional quieting (blocked dopamine)
Improved mood (increase serotonin)
Tx + and - symptoms of schizophrenia
Clozapine
Side Effects
Bone marrow suppression
Agranulocytosis
Can be fatal
Risperidone
2nd Gen Antipsychotics (SGA)
Paliperidone
2nd Gen Antipsychotics (SGA)
Quetiapine
2nd Gen Antipsychotics (SGA)
Olanzapine
2nd Gen Antipsychotics (SGA)

CRAPiZgOzQs
2nd Gen Antipsychotics
CLOZapine
RISPERidone
Aripiprazole
Paliperidone
Ziprasidone
Olanzapine
Quetiapine
2nd Gen SE
Fewer EPSEs
Metabolic syndrome - weight gain, hyperglycemia/insulin resistance, triglycerides
Sexual dysfunction
NMS
Risperidone SEs
Highest risk for EPSEs
⏫ Prolactin
Orthostatic hypotension
Wt gain
Sedation
Sexual dysfunction
Paliperidone SEs
similar to Risperidone
Quetiapine SEs
Strong sedation and wt gain
Olanzapine SEs
Highest SGA risk for metabolic syndrome, highly sedating (street value for this)
Ziprasidone SEs
Prolongs QT interval, used for emergencies
Aripiprazole SEs
Insomnia, akathisia, low risk for metabolic syndrome, wt neutral and less hyperprolactinemia and sedation
Benztropine (anticholinergic) SEs
Dry mouth
Blurred vision
Constipation
Urinary retention
Orthostatic hypotension
Benztropine (anticholinergic) Use
Preventative or acute tx of EPSEs (dystonia) - restores natural balance of acetylcholine and dopamine
