What are indications of dabigatran?
Treatment of DVT & PE

What is the MOA of dabigatran?
Direct inhibitor of thrombin
What are the advantages of dabigatran( Pradaxa)?
Rapid onset
No anticoagulation monitoring
Few drug-food interactions
Lower risk of major bleeding
What are the disadvantages of dabigatran (Pradaxa)?
Dosing on time is important
THERE IS NO ANTIDOTE
What is the regular dosing for aspirin?
81 mg for chronic use/old people
325 mg for initial treatment
What are indications for aspirin?
Ischemic stroke TIA's Chronic stable angina Unstable angina Coronary stenting Acute MI
What is the MOA of Aspirin?
Suppresses platelet aggregation by causing irreversible inhibition of COX.
What are the adverse effects of Aspirin?
Risk for GI bleeding
Hemorrhagic stroke
What should the nurse include in the plan of care for patients on aspirin?
Bleeding precautions
Do not administer other NSAIDS
Monitor for tarry stools
What is the regular dose of Clopidogrel (Plavix)?
75 mg
What are indications for Clopidogrel (Plavix)?
Preventing blockage of coronary artery stents
Reduction of thrombotic events in patients with ACS
What is the MOA of Clopidogrel (Plavix)?
Blocks ADP receptors on platelets and thereby prevents ADP-stimulated platelet aggregation
What kind of drugs interact with Clopidogrel (Plavix)?
Drugs that promote bleeding

What are nursing considerations for Clopidogrel (Plavix)?
Determine date of last use is patient is scheduled for a surgical procedure. Withdraw 5 days before surgery.
