Some gram positive
Some gram negative
some anaerobes
Broad Spectrum
MOA of Tetracyclines
Inhibition of protein synthesis
Bacteriostatic
Can be bacteriocidal at higher doses and depending on microorganism
Kinetics of Tetracyclines
Variable liver metabolism
Renal excretion
When given PO on empty stomach, 75% absorbed
Cautions with Tetracyclines
In children younger than 8 (prevents development of teeth)
Hepatic and Renal patients
Contraindications with Tetracyclines
ALlergy to Tetracyclines
Pregnancy F and Lactation (crosses placenta and breast milk)
Adverse effects with Tetracyclines
Tooth discoloration Photosensitivity -wear sunscreen GI upset Rash Ototoxic - usually reversible Can decrease effects of oral contraceptives High potential to cause super infections
Drug interactions with Tetracyclines
PCN G - Decrease effectiveness; may need to be increased PCN G if on combo
Oral contraceptives
Antacids
Calcium - binds to calcium and then calcium cant do what its supposed to do
Warfarin
Education with Tetracyclines
Keep out of reach of children
Take on and empty stomach and not with dair
Write expiration date - decompose and become toxic to kidneys
Take antacids or OTC meds that contain mineral salts at least 2-3 hours after tetracycline
Advise women on the pill to use back up method
Wear sunscreen (photosensitivity
Dont use Tetracycline eye drops with contacts - will damage contacts
