- Minor - aim INR <3.5
2. Major - stop warfarin 3-5 days pre-op

A
Vitamin K +/- FFP or prothrombin complex concentrate.

How do you manage anticoagulation for a patient on warfarin before elective surgery?
A
Stop 5 days before, convert to LMWH and stop this 6 hours before, monitor aPTT.
