Anticoagulant Therapy Flashcards
Antidote for warfarin
vitamin K
INR lab values
Normal < 1.1
Therapeutic: 2.0-3.0 (A.fib, DVT, PE)
High-risk patients: 2.5-3.5 (mechanical valves)
INR indication
INR is used to measure the PT ratio and warfarin therapy.
normal aPTT
23.3-31.9 seconds
Prolonged PTT
Prolonged PT means that the blood is taking too long to form a clot.
Clinical value for aPTT with people when the client is receiving heparin therapy
1.5-2.5 times longer than the person’s pretreatment level
Antidote for heparin
protamine sulphate
Partial Thrombin Time (PT) therapeutic value of a client taking warfarin
17.25-25 seconds
Normal PTT intervals
11-12.5 seconds
aPTT value for someone on a heparin infusion
34 to 78 seconds
aPTT evaluates this medication and sequence.
aPTT evaluates how well the coagulation sequence is functioning by measuring the amount of time it takes in seconds for recalfiied citrated plasma to clot after partial thromboplastin is added to it
- monitors the effectiveness of heparin
- screens to coagulation disorders
when should blood be drawn to measure aPTT?
1 hour prior to heparin dose
if aPTT value is greater than 100 seconds, what should you do?
initiate bleed precautions
What does PT measure?
PT measures the amount of time it takes in seconds for clot formation and is used to monitor response to warfarin sodium therapy
- screens for extrinsic clotting disorders (vitamin K deficiency, DIC)
normal INR interval
<1.1
