loading dose for unfractionated heparin (UFH) in UA/NSTEMI
60units/kg bolus
maintenance dose of UFH in UA/NSTEMI
12 units/kg/hr for 48 hours or until PCI
UFH loading dose in STEMI
fibrinolytic 60 units/kg bolus
UFH maintenance dose in STEMI
12 units/kg/hr until PCI
UFH dosing in PCI
with GPIIb/IIIa 50-70 units/kf bolus
without GPIIb/IIIa: 70-100 units-kg IV bolus
until PCI
enoxaparin dosing for UA/NSTEMI
1mg/kg every 12 hours adjusted for CrCl less than 30ml/min until discharge or PCI (up to 8 days)
enoxaparin dosing for STEMI
less than 75 years: 30mg IV then 1mg/kg every 12 hours
older than 75 years: .75mg/kg every 12 hours
until discharge or PCI (up to 8 days)
enoxaparin dosing for PCI
administer additional 0.3mg/kg IV if last dose is 8-12 hours before PCI
bivalirudin indication
only to be used in patients with planned early invasive strategy
