Anticholinesterase Agents Flashcards
Competitive (reversible) inhibition of AChE
- block ACh degradation, enhances effects of ACh
- at high doses, AChE can produce effects at all sites where ACh is NT (muscarinic, ganglionic, NMJ, CNS)
- at therapeutic doses, effects limited to muscarinic receptors at NMJ
Prototype anticholinesterase
Neostigmine
Neostigmine - Pharmacological Effects
muscarinic response neuromuscular response low dose - increase contraction high dose - reduce contraction **only inhibitor that is also an agonist at NMJ nicotinic receptors**
What is the only AChE agent that is also an agonist at NMJ nicotinic receptors?
neostigmine
Neostigmine - therapeutic uses
Myasthenia gravis
reversal of competitive neuromuscular blockade
postop bladder distension/urinary retention
Neostigmine - Side Effects
excessive muscarinic stimulation (treat with atropine)
neuromuscular blockade, respiratory depression @ toxic dose
Neostigmine - Contraindications
asthma, peptic ulcers, _____
Physostigmine - drug classification
AChE inhibitor
Physostigmine - actions
crosses BBB
no direct agonist activity
Physostigmine - Uses
Treats atropine poisoning and other drugs that cause central/peripheral muscarinic blockade
Donepezil - drug classification
AChE inhibitor
Donepezil - therapeutic use
cognitive decline in Alzheimer pts
- improves cognition, not disease-modifying
Organophosphorus Compounds are __________ inhibitors
irreversible
OP compound prototype
malathion
Malathion - characteristics
high lipid solubility
absorbed via all routes, skin included
What is the common source of accidental/agricultural poisoning?
Malathion, irreversible inhibitors
Malathion - therapeutic use
head lice
What is echothiophate occasionally used to treat?
open-angle glaucoma
OP Poisoning Diagnosis (Cholinergic crisis)
M: sweating, salivation, miosis, urination, defecation
N: muscle weakness, fasciculation, paralysis
CNS: confusion, ataxia, convulsions, coma, respiratory depression
What clinical test can be used to detect OP poisoning?
Depressed plasma cholinesterase activity
OP Poisoning - Treatment
Atropine: large doses, short intervals, blocks M
Pralidoxime: reactivates AChE only at NMJ (MUST admin before “aging”)
