Gentamicin

A

Use ideal or actual weight whatever is less.

Use Adjusted in Obesity. Adjusted weight = 0.4(actual weight – ideal weight) + ideal weight). Seek specialist advice if BMI >35.

Adult

Use the higher dose for young adults and the lower one for the elderly.

CrCl >60 mL/minute, IM/IV 5–7 mg/kg once daily.

CrCl 30–60 mL/minute, IM/IV 4–5 mg/kg once daily.

CrCl <30 mL/minute, seek specialist advice.

Child

1 month – 10 years, IM/IV 7.5 mg/kg (maximum 320 mg) once daily.

> 10 years, IM/IV 6–7 mg/kg (maximum 560 mg) once daily.

Endocarditis

IV, 1 mg/kg every 8 hours (with other agents). This dose is not used for empirical treatment.

Surgical prophylaxis

Adult, IV 2 mg/kg within 2 hours before incision. Give 5 mg/kg for procedures likely to continue for >6 hours (eg cardiac surgery).

Amikacin

A

Adult
Use the higher dose for young adults and the lower one for the elderly.

IM/IV, initially 15–20 mg/kg once daily. Doses of 25–30 mg/kg once daily may be needed in critically ill patients.

Non-tuberculous mycobacterial infections

Use with other anti-infective agents.
Daily regimen, IM/IV, initially 10–15 mg/kg once daily.
Three times a week regimen, IM/IV, initially 15–25 mg/kg once daily 3 times a week.

TB
Use with other anti-infective agents.

IM/IV, initially 10–15 mg/kg (maximum 1 g) once daily on 5–7 days a week, then 15–25 mg/kg once daily 3 times a week.

Child
1 month – 10 years, IM/IV, initially 22.5 mg/kg (maximum 1.5 g) once daily.
>10 years, IM/IV, initially 18 mg/kg (maximum 1.5 g) once daily.

Inhalation

Adult, child >6 years, neb 300 mg twice daily for 28 days; stop for 28 days, then keep repeating cycle.

Adult, child >6 years, DPI 112 mg (4 capsules) twice daily for 28 days; stop for 28 days, then keep repeating cycle.
