Abx: Cryptococcus
Pulmonary: Fluconazole
Meningitis/Severe Pulmonary: Amphotericin B + Flucytosine
Abx: CMV
Gancyclovir
Abx: Encephalitis
HSV or Undiff: Acyclovir
CMV: Gancyclovir and Foscarnet
If chance of ricketssial: Add Doxy
Abx: Endocarditis
IVDU/Acute Native/Prosthetic >1 yo: Vanc + Cefepime
Prosthetic < 1 yo: Vanc + Cefepime + Rifampin + Gent
Native (SubAcute): Vanc + Rocephin
Abx: Endometritis
Clinda
Abx: Epiglottitis
Rocephin or Unasyn
Abx: Diverticulitis
Cipro/Flagyl
Abx: Neutropenic Fever
Cefepime +/- Vanc
Abx: Malaria
Ovale, Vivax, Malarie - Chloroquine if sensitive
Falcip, Chloriquine Resistance - Artesunate
Pregnant - Quinine + Clinda
Abx: Meningitis
< 1 mo: Amp + Gent
1mo - Adult: Vanc + Rocephin +/- Acyclovir
Pen Head Inj/Shunt/Post Sx: Vanc + Cefepime
Abx: Ludwig’s
Unasyn (Amp/Sulbactam) or Clinda
Abx: Intracranial Abscess
Vanc + Rocephin + Flagyl
Abx: Mastoiditis
Mild: AOM Tx (Augmentin)
Severe: Vanc + Rocephin
