Epiglottitis/Supraglottitis:
- Cextriaxone
Pneumonia
- CAP:
Mild/Moderate: Amoxicillin
Severe: Co-amoxiclav + Doxycycline
ICU/HDU/NBM: Co-amoxiclav + Clarithromycin
- HAP:
Non-Severe: PO Amoxicillin
Severe: IV Amoxicillin + Gentamicin
- Aspiration
Non Severe: Amoxicillin + Metronidazole
Severe: Amoxicillin + Metronidazole + Gentamicin
- Acute Exacerbation of COPD (or acute bronshitis/cough in the elderly):
Amoxicillin (doxy if no effect)
Heart
- Endocarditis
Native Valve (indolent/subacute): amoxicillin + gentamicin
Native Valve (severe/Sepsis): Flucloxacillin
Prosthetic Valve or Suspected MRSA:
Vancomycin and Gentamicin
GI
- C.Diff
Non Severe: Metronidazole
Severe: Vancomycin +/- Metronidazole
- Peritonitis/Biliary Tract/Intra-abdominal
IV amoxicillin + Metronidazole + Gentamicin
- Proven Spontaneous Bacterial Peritonitis
Mild: Co-trimoxazole
Severe: Piperacillin/Tazobactam
GU
- Uncomplicated Female Lower UTI OR Uncatheterised Male UTI:
Nitrofurantoin or Trimethoprim
- Complicated UTI/Pyelonephritis/Urosepsis:
Amoxicillin + Gentamicin
Bone/Skin
- Cellulitis/Acute Septic Arthritis/Osteomyleitis:
Flucloxacillin
- Open Fracture Prophylaxis:
Cefuroxime
- Diabetic Foot Infection:
Mild: Flucloxacillin or Doxycyxline
Moderate: Flucloxacillin or Doxycycline AND Metronidazole
Severe Systemic Infection Unknown Source
Amoxicillin + Metronidazole + Gentamicin
if Pen allerg:
Vancomycin + Metronidazole + Gentamicin
