What defines a mild community acquired pneumonia and how do you treat it?
CURB 65 = 0-2
(confusion, urea > 7, RR > 30, BP <90-systolic / <60-diastolic, age>65)
Amoxicillin PO (doxycycline if allergic)
What defines a severe community acquired pneumonia and how do you treat it?
CURB 65 > 3
(confusion, urea > 7, RR > 30, BP <90-systolic / <60-diastolic, age>65)
Co-amoxiclav IV (clarithromycin if allergic)

Doxycycline IV
Patient presents with mild community acquired pneumonia but is taking nill-by-mouth. How do you treat them?
IV clarithromycin
How do you treat severe hospital acquired pneumonia?
IV amoxicillin (co-trimoxazole if allergic)
IV metronidazole
IV gentamicin
How do you treat non-severe hospital acquired pneumonia?
PO amoxicillin (co-trimoxazole if allergic) PO metronidazole
What are the indications of antibiotic treatment in an acute exacerbation of COPD and how do you treat it?
Increased suptum purulence
Amoxicillin (doxycycline if allergic)
What is the initial management of suspected endocarditis?
3 blood cultures
3 as bacteraemia is constant in endocarditis and would expect to see all 3 positive in endocarditis
If negative but still strong clinical suspicion, do serology for atypical organisms (e.g. HACEK)
How do you treat subacute/indolent endocarditis?
Amoxicillin and gentamicin IV
How do you treat endocarditis associated with sepsis?
IV flucloxicillin
How do you treat endocarditis in a patient with prosthetic valves?
Vancomycin, gentamicin and rifampicin IV
How do you treat endocarditis in a patient with suspected MRSA?
Vancomycin, gentamicin and rifampicin IV
same as prosthetic valve
How do you treat severe and non-severe clostridium difficile infection?
Non-severe = metronidazole PO
Severe - vancomycin PO (+/- IV metronidazole)
How do you define severe and non-severe c. difficile infection?
Severe is at least one of
- temperature >38.5
- colonic dilation
- WBC >15x10^9
- creatitine >1.5 x baseline
- persistent
How do you treat uncomplicated UTI in males?
Nitrofurantoin or trimethoprim for 7 days
How do you treat uncomplicated UTI in females?
Nitrofurantoin or trimethoprim for 3 days
How do you treat complicated UTI?
IV amoxicillin (co-trimoxazole if allergic)
and
IV gentamicin
What antibiotic is used to treat cellulitis?
Flucloxicillin (doxycycline if allergic)
What antibiotic is used to treat septic arthritis/osteomyelitis?
Flucloxicillin
What antibiotics are given as open fracture prophylaxis?

What defines mild, moderate and severe diabetic foot ulcer and how are they each treated?
Mild
- localised infection symptoms /cellulitis within 2cm
- flucloxicillin
Moderate
- infection spreading to soft tissues around ulcer/cellulitis outwith 2cm but to systemic symptoms
- flucloxicillin and metrodidazole
Severe
- signs of sepsis
- flucloxicillin, gentamicin/aztreonam, metronidazole
What antibiotics are used in necrotising fasciitis?
Flucloxicillin, clindamycin, gentamicin
What antibiotics are used in tonsillitis?
Penicillin (clarithromycin if allergic)
What antibiotics are used in quinsy?
Penicillin (clindamycin is allergic)
