Special rule for Gentamicin
If IV therapy is still indicated after 72 hours of Gent:
- Check microbiology results and sensitivities
- Consider switch to Aztreonam
ENT:
Epiglottitis Treatment
IV Ceftriaxone
LUNG:
CAP - moderate
CURB65
IV/PO Amoxicillin
Penicillin Allergy - PO Doxycycline on day 1 then IV Clarithromycin
Total treatment period = 5 days
LUNG:
CAP - severe
IV Co-amoxiclav + PO Doxycycline
If they are in ICU, instead of Doxy, use IV Clarithromycin
Step down to Doxycycline for ALL patients
If penicillin allergy - IV Levofloxacin
Total treatment period = 7 days
LUNG:
HAP - moderate
PO Amoxicillin + Metronidazole
If penicillin allergy - PO Co-trimoxazole + Metronidazole
Total treatment period = 5 days
LUNG:
HAP - severe
IV Amoxicillin + Metronidazole + Gentamicin
Penicillin allergic - IV Co-trimoxazole + Metronidazole +/- Gentamicin
Step down to Co-trimox + Metro
Total treatment time = 7 days
LUNG:
Acute exacerbation of COPD
1st line - Amoxicillin
2nd line - Doxycycline
When do you give antibiotics during an acute exacerbation of COPD
Increased sputum purulence
Consolidation of CXR
LUNG:
Acute cough/acute bronchitis
1st line - Amoxicillin
2nd line - Doxycycline
When do you give antibiotics during an acute cough/acute bronchitis
If frail or elderly
Otherwise, antibiotics give no significant benefit
HEART:
Endocarditis - Native Valve indolent (Sub acute)
(Indolent means it causes little or no pain)
IV Amoxicillin + IV Gentamicin
HEART:
Endocarditis - Native valve severe sepsis (Acute)
Flucloxicillin
HEART:
Endocarditis - Prosthetic valve or suspected MRSA
IV Vancomycin + IV Gentamicin + PO Rifampicin
HEART:
Endocarditis - Native valve sever sepsis + risk factors for resistant pathogens
IV Vancomycin + IV Meropenem
GI:
C. diff - non severe
PO Metronidazole
Treatment period = 10 days
GI:
C. diff - severe
PO Vancomycin +/- IV Metronidazole
Treatment period = 10 days
GI:
Peritonitis/Biliary Tract Infection/Intra abdominal infection
IV Amoxicillin + Metronidazole + Gentamicin
then step down to PO Co-trimox + Metro
If penicillin allergic: IV Vancomycin + Metronidazole + Gentamicin
then step down to PO Co-trimox + Metro
Total treatment period = 7-10 days
GI:
Proven spontaneous bacterial peritonitis - mild
PO Co-trimoxazole
Total treatment time = 5-7days
GI:
Proven spontaneous bacterial peritonitis - severe
IV Piperacillin/Tazobactam
then step down to PO Co-trimoxazole
Total treatment time = 5-7 days
RENAL:
Complicated UTI / Pyelonephritis / Urosepsis
IV Amoxicillin + Gentamicin
then step down to PO Co-trimoxazole
If penicillin allergic - IV Co-trimoxazole + Gentamicin
Total treatment time = 5-7 days
BONE/SKIN:
Cellulitis
Flucloxicillin
If penicillin allergic - Doxycycline
Total treatment time = 7 days
BONE/SKIN:
Diabetic foot infection - mild to moderate
Flucloxicillin
If penicillin allergic - Doxycycline
Add in metronidazole if you think its worse
Total treatment time = 7days

IV Co-trimoxaclav/Co-trimoxazole + Metronidazole
