- known or suspected neutropaenia

Blood Pressure: systolic
Mild/Moderate CAP (0-2)
Amoxicillin IV/PO (pen allergic Doxycycline PO)
Severe CAP (3-5)
Co-amoxiclav IV + either Clarithromycin IV or Doxycycline PO (if pen allergic IV Levofloxacin)
Severe HAP
IV Amoxicillin + Metronidazole + Gentamicin (pen allergic: IV Co -trimoxazole)
Non Severe HAP
PO Amoxicillin + Metronidazole (5 days) (Pen allergic: Co-trimoxazole)
Acute Exacerbation of COPD
ABs if purulent sputum. If no purulent sputum then no ABs, unless CXR shows consolidatio or signs of pneumonia.
Native Valve indolent Endocarditis
Amoxicillin IV + Gentamicin
Native Valve Acute Endocarditis
Flucloxacillin IV
Prosthetic Valve Endocarditis
Vancomycin IV + Rifampicin PO + Gentamicin IV
Native valve severe sepsis + risk factors for resistant pathogens
