Esmolol - Class
Class II antiarrhythmic beta blocker
Esmolol - Action
Slows HR through selective blockade of beta 1 receptors; short half-life
Esmolol - Indication
Tachycardia
Esmolol - Contraindications
Asthma, heart block, bradycardia, cardiogenic shock
Esmolol - Dose & Route
50-100 mcg/kg/min; IV
Esmolol - Adverse
Bradycardia, hypotension, CHF, lethargy
Esmolol - Other
Hypotension is common. Do not admin IV to PT’s w/ Calcium channel blockers.
Labetalol - Class
Class II antiarrhythmic beta blocker
Labetalol - Action
Lowers BP through nonselective blockage of beta receptors and limited blockage of Alpha 2 receptors
Labetalol - Indication
Hypertensive emergency
Labetalol - Contraindications
Asthma, heart block, bradycardia, cardiogenic shock
Labetalol - Dose & Route
10-20mg; IV
Labetalol - Adverse
Bradycardia, hypotension, CHF, lethargy
Labetalol - Other
Should not be administered to patients receiving IV calcium channel blockers
Diltiazem - Class
Class IV antiarrhythmic, calcium channel blocker
Diltiazem - Action
Lowers BP by relaxing the vascular smooth muscle and slows AV conduction
Diltiazem - Indication
Rapid ventricular w/Afib, stable narrow complex tachyarrhythmias
Diltiazem - Contraindications
Hypotension, CHF, cardiogenic shock, wide complex V-Tach
Diltiazem - Dose & Route
15-20mg; IV
Diltiazem - Adverse
N/V, dizziness, headache, hypotension
Diltiazem - Other
Can be given as IV bolus or IV infusion, calcium chloride can reverse some of the untoward effects
Magnesium Sulfate - Class

Magnesium Sulfate - Action
Physiologic calcium channel blocker and bronchodilator
Magnesium Sulfate - Indication
Torsades des pointes, asthma, hypertensive disorders of pregnancy
